We are proud that you have chosen to enroll your child in our district.  Our schools, Columbia Elementary and Mountain View Middle, are safe, well maintained, exciting, stimulating and success oriented.  Our district Mission is to ensure learning for all children - no limits, no excuses.  We do this by building positive relationships, maintaining high expectations, creating relevance, providing more time and support, meeting the physical, intellectual, emotional, social and moral needs of our children, and by being persistent and passionate about children.
 
The following packet includes all of the enrollment pages you will need to enroll your child in the Columbia School District.  All parents are required to have their child’s current immunization records and verification of age when they enroll.
 
All families are required to provide proof of residency (current utility bill or rental agreement). Families residing outside of our district will also be required to obtain an Interdistrict Transfer request from your district of residence before we can complete the Columbia School District enrollment process.

TK & Kindergarten Requirements

Children MUST be current on their immunizations.
 
Children entering Transitional Kindergarten or Kindergarten must have:
5 DTaP (4 doses meet requirement if at least one was given on or after the 4th birthday)
4 Polio (3 doses meet requirement if at least one was given on or after 4th birthday)
3 Hep B
2 MMR
2 Varicella
 
Under the law know as SB277, beginning January 1, 2016 exemptions based on personal beliefs, including religious beliefs, will no longer be an option for the vaccines that are currently required for entry into child care or school in California.

1st Grade Health Exam

California State Law (Health and Safety Code 124085) requires all children entering FIRST GRADE to have a complete health examination.  Kindergarten students may obtain this exam 18 months prior to entering First Grade.

7th Grade Requirements

All students entering, advancing or transferring into 7th grade need proof of an adolescent whooping cough booster immunization (called “Tdap”).

This school immunization law affects all students in public and private schools.

Pertussis is a very contagious respiratory disease that can be severe and last for months. The immunity received from either early childhood immunization or pertussis disease wears off over time, leaving older students and adults susceptible again to pertussis. Immunization with Tdap can protect students, schools and communities against pertussis.

Please check with your Primary Care Provider to schedule your child’s Tdap Booster shot.  Please bring the proof of immunization to your child’s school office.
